Position Title

    Rapid Re-Housing Case Manager   


    The Rapid Re-Housing Case Manager provides financial support and case
    management services to eligible individuals and families to move them from an
    experience of homelessness to housing. The Rapid Re-Housing Program is part of
    a collaborative effort to create supportive housing opportunities for families and
    individuals experiencing homelessness to achieve the goal of ending homelessness
    by quickly increasing the housing stability of homeless households.


    A. Outreach and Screening
        1. Work directly with coordinated entry system to identify potential program
        participants per contract requirements.
        2. Develop and maintain relationships with applicants to facilitate the screening
        and intake processes.
        3. Provide timely screening of potential participants to determine program
        4. Clearly communicate reasons for ineligibility to all applicants.
        5. Work in collaboration with partner agencies to educate housing providers
        about the program and develop relationships with potential landlords.
        6. Engage in apartment search with eligible participants, acting as the liaison
        to landlords to address landlord concerns.
        7. Refer ineligible applicants to other potential resources.

    B. Landlord Liaison
        1. Work with partner agencies to identify potential landlords in community of
        participant’s choice.
        2. Engage landlords in educational dialogue about common issues facing
        homeless youth and how the RHH program helps to mitigate these issues.
        3. Work in collaboration with partner agency to ensure that residential
        habitability inspections are satisfactorily completed.
        4. Complete all necessary paperwork with the participant and landlord.
        5. Maintain relationship with landlords as their first point of contact regarding
        participant tenancy issues.
        6. Process rental expenses monthly and ensure landlords receive timely        

    C. Supportive Services
        1. Provide case management services as needed to participants in housing.
        Generally, these services are provided in the participant’s home.
        2. Engage participant in goal planning around employment, education, life
        skills, mental and emotional health, physical health, permanent housing plan,
        support systems, and other individual goals.
        3. Provide resources and referrals to community organizations and assist
        with application for benefits.
        4. Meet with supervisor weekly to discuss referrals, landlord issues and staff
        current caseload.

    D. Interagency Collaboration
        1. Participate in monthly collaborative meetings with partner agencies in an
        effort to contribute to group learning.
        2. Work collaboratively with partner agencies to develop policies and
        procedures, language and outreach materials.
        3. Work with partner agencies to develop a working list of partner landlords.

    E. Data Collection, Recordkeeping and Reporting
        1. Maintain updated and accurate participant files.
        2. Maintain detailed documentation and recordkeeping to meet all contract
        and grant requirements, outcomes, and audits.
        3. Submit purchase requests, reports and billing information to the Program
        Manager as required by contracts and Friends of Youth.
        4. Ensure all requirements of the Rapid Re-Housing Program are met.

Required Skills and Qualifications

    A. Education and Experience
        1. Bachelor's degree and a minimum of 2 years equivalent work experience.
        2. Prior experience addressing youth, young adult and family homelessness
        3. Prior experience providing case management services.   
        4. Experience with motivational interviewing concepts and employment
        coaching preferred.

    B. Knowledge, Skills and Abilities
        1. Demonstrate sensitivity and responsiveness to cultural differences and a
        commitment to the value of cultural competency.
        2. Must be able to establish rapport with at-risk young adults.
        3. Ability to maintain professionalism in controversial, complex and public
        4. Strong awareness of ethical issues and boundaries.
        5. Excellent interpersonal, writing, record-keeping and organizational skills.
        6. Self-starter with ability to work independently and on collaborative teams.
        7. Strong computer skills using Windows Word, Outlook and Excel programs.   
        8. Employees and volunteers who operate their own or Friends of Youth vehicles
        on Friends of Youth business must have a safe driving record as defined by
        Friends of Youth.
        9. Employees and volunteers who operate their own vehicle(s) on Friends of
        Youth business must carry auto liability insurance that complies with the
        standards of Friends of Youth.
        10. Knowledge of trauma-informed care, harm reduction philosophy and
        strengths-based practice preferred.


    Full Time


    $19 - 21 / hour DOE
